ALBANY – Grants to fund healthcare restructuring projects include a number in the Hudson Valley and Catskills.

The funds are meant to expand the availability of primary care in local communities, cut duplicative services in hospitals, right-size nursing homes and fund consolidation projects to health care providers.

St. Luke’s Cornwall Hospital will receive $19.4 million for capital restricting projects for consolidation of inpatient services. Elant at Brandywine, Inc. will receive $1.4 million to decertify 131 beds and close Elant at Brandywine. Bethel Nursing Home, Inc. will receive $67,000 for home care expansion.

Funding for healthcare expansions include:

• A.O. Fox Memorial Hospital Society, $500,000 – Delhi Primary Care
• Ezras Cholim Health Center, Inc., $614,000 – upgrade the medical records system
• Hudson River HealthCare, Inc., $3.4 million – Amenia expansion of existing primary care capacity
• Hudson River HealthCare, Inc., $500,000 – Haverstraw expansion of existing primary care capacity
• Mount Vernon Neighborhood Health Center, Inc., $460,000 – Open Door Family Medical center’s primary care expansion project
• Refuah Health Center, $1.4 million – Expansion of primary care services at Spring Valley site
• The Center for Recovery -- $693,000 – The Discovery Health Center – Medical Home for Hudson Valley’s developmentally disabled
• The Westchester Institute for Human Development, $$363,000 – Using telemedicine to enhance primary care access and effectiveness
• Valentine Lane Family Practice, $1.8 million – Valentine Lane Family Practice expansion
